Seniors:
If you need a copy of your transcript sent to colleges, universities
or other organization requesting the information, please visit the
Career Center and fill out a "White Transcript Request Card."
For a "Final Transcript Request" to be sent to colleges
and universities, please see Mrs. Holman in the Registrar's Office
before you graduate in June.
Freshman, Sophomores, and Juniors:
If you need a copy of your transcript, please see Mrs. Holman in

AHS Graduates:
If you need a copy of your transcript, please see Mrs. Holman in

Please note that the forms are in PDF format. You

In order to fill out the forms, we recommend that
you save them to your computer. You can type directly into the form
and then print it out when done. Because these forms require a signature,
we cannot accept e-mailed forms. However, you can fax them to us at
