Back to Registrar main pageWithdrawal ProcedureRequest to Withdraw Form1. Parent/guardian completes the “Request to Withdraw” form.2. Parent/guardian must physically turn the form in to the registrar. 3. If the parent/guardian is not physically present to withdraw the student, the registrar will contact the parent/guardian via information already on the file at the school to verify the authenticity of the request prior to proceeding with the withdraw process. Withdrawal Form1. On the student’s last day of school at AHS, he/she will route the withdraw form to receive exit grades and turn in books.2. When the withdraw form is completed the student will turn the form in at the registrar’s office. 3. The registrar will prepare a packet for the student to take to their new school. The signed “Request to Withdraw form” by the student’s guardian and the “Withdraw form” the student routes to their teacher must be returned to the Registrar to complete the withdraw process.
