Athlete Physical Form

Sports Physical Requirement
All students participating in athletics must have a current sports physical on file with the school. The physical form must be completed, signed by a licensed medical provider, and stamped with the clinic stamp. Forms that are missing either the provider’s signature or the clinic stamp cannot be accepted.

Sports physicals are valid for 24 months from the date of the exam. Please check with your school’s athletic office if you are unsure whether your student’s physical is current.
