Before turning out for a sport, you must purchase an ASB Card ($45.00), clear all fines and complete an Athletic Packet. Forms will now be ALL ONLINE! Beginning August 1st parents and athletes will log on to SKYWARD to complete the necessary forms. The only hard copy form will be the actual Doctor signed physical form. Physicals are good for two years. If you are unsure of when your physical expires, you can check your status in the Athletics Office. All athletes with a valid physical, completed parental packet and ASB card must obtain an Athletic (yellow) Card from the Activities Office before turning out. No card -- No play!Athletes who do not complete all forms WILL NOT be eligible to practice/tryout until they have done so!
