School-Based Health Center Survey
The ASD is considering providing a school-based health center. We are asking families to complete a survey so we can better understand students’ healthcare needs and what we need to know as we plan a school-based health center. School-based health centers offer comprehensive health care to PK-12 students. They are typically staffed by nurse practitioners, coordinators and mental health counselors. They offer services such as diagnosis and treatment of minor illnesses and injuries, management of chronic health conditions such as asthma, sports physicals, immunizations, reproductive health and mental health counseling. Services are provided regardless of a student’s ability to pay and increase access to health care for our students.
We are collecting input about the possible School-Based Health Center to evaluate students’ health care needs. All answers are anonymous. Please complete this survey by May 20. If you have questions, please contact Ashley Boyd in the Department of Family Engagement and Student Success.
